Rod spec sheets for customer rods

I know I have seen a copy of a sheet that records the customers name and rod build specs used for each custom blank on this forum. But I cannot find a copy of it anywhere on my PC. Could someone please repost a copy of what they use when building customer rods. Im wanting to start keeping this data on the rods I build. I have received some positive feedback on several of my rods, and I find myself clueless on the exact layout i used for these rods that Im wanting to duplicate.

We have a rod spec/order sheet that we use for all our rods, including repair/rehab/refurbish as well as new custom. I can mail you one is you like.

Brad, If you want to go a step further you can record some critical data on a small sheet of paper and insert it into the butt end of the rod before you apply the butt cap. I usually write down the rod specs, guide type, thread colors, and date of build. I roll up the paper, tie a small piece of cord around it, and leave a small tag end just under the butt cap. If I ever get the rod back for repairs in the future I'll have all the data I need to repair/rebuild it.

Thanks Everyone!! Looking foward to this years show, it is so nice to be in Raleigh and have High Point in the backyard. But Im still planning on staying overnight and making a weekend out of it. See yall at the show!!
